=== Respiratory === The primary cause of respiratory failure in dermatomyositis is interstitial lung disease, resulting from damage to the lung interstitium. This is particularly prominent in patients with anti–MDA5 autoantibodies, who are at high risk of developing rapidly progressive interstitial lung disease. In some people, the condition affects the diaphragm muscle, the lungs directly (through inflammation), or both.
